Social Worker – Spanish Bilingual

Meals on Wheels San Francisco works to help seniors live independently, in their own homes, for as long as it is safely possible. The Social Work department plays an integral role in helping seniors stay healthy and safe.

 

The Social Worker conducts initial and annual in-home eligibility assessments with homebound older adults, provides information and referral services as needed, and works closely with other staff to ensure smooth delivery of services to clients. The social worker establishes and maintains professional relationships with other care providers, case managers, county agencies, community resources, clients and families. The Social Worker may occasionally handle urgent or complex client situations as Social Worker of the day and/or provide some short-term case management services after six months on the job.

 

Social Workers demonstrate in his/her performance, adherence to established policies and procedures exhibiting the characteristics associated with good-judgement, clear and considerate communication, and collaboration.

DUTIES AND RESPONSIBILITIES:

Under the supervision of the Social Work Supervisor in accordance with the provisions of the MOWSF’s Employee Handbook, and the Social Work Procedures Manual, the Social Worker is responsible for:

  • Conducting 40 in-home initial and annual assessments monthly requiring a high degree of technical competence. Requires traveling throughout all areas of San Francisco to determine eligibility for the home delivered meal program, meal type, and other appropriate services.
  • Responsible for assessments of Spanish monolingual clients.
  • Establishing and maintaining appropriate confidential electronic records by entering client data into MOWSF Salesforce and Department of Aging CAGetCare databases as required.
  • Making appropriate referrals to community resources and coordinating services for to positively impact client care.
  • Assisting with development and maintenance of information on community resources.
  • Keeping updated with and carrying out all MOWSF agency, social work, and client policies.
  • Responding to client inquiries about the program and handling client situations as they arise.
  • Working with the Home Delivered Meal staff, driving staff, client needs, and dieticians, to ensure clients are effectively served.
  • Meeting with drivers weekly to monitor clients’ eligibility and safety, complete quarterly reassessments and address any other concerns or needs.
  • Following up with clients not answering the door for their meal delivery.
  • May provide short-term case management to a small caseload of clients.
  • Referring clients to MOWSF volunteer services, handling client/volunteer concerns as needed.
  • Attending relevant community senior and social service meetings as requested and/or assigned.
  • Participating in quarterly peer record review
  • Working at MOWSF events as requested such as Thanksgiving Day and annual Gala event.
  • Completing monthly reporting requirements.
  • Delivering meals in emergency situations as needed.
  • Performing other duties as assigned.

 

JOB REQUIREMENTS:       

A Bachelor’s degree in Social Work, Psychology, Gerontology or related field is required

  • 2 years relevant work experience preferred
  • Bilingual in Spanish is required
  • Knowledge of community resources in San Francisco
  • Manages time by prioritizing assignments and tasks within established deadlines
  • Maintains data integrity while adhering to strict client confidentiality policies
  • Excellent organization, data entry, and written documentation skills
  • Aptitude for computers - Microsoft Office software and Salesforce experience preferred
  • Skilled in interpersonal communication, interview and assessment, conflict de-escalation and resolution
  • Ability to work:

o   with a diverse community
o   in an independent and self-directed manner with minimal supervision

  • Valid driver’s license, proof of insurance and automobile required

 

SALARY / HOURS / BENEFITS:                                

  • The position is hourly and based on a 40-hour work week with a 30 minute paid lunch break.

Requirements
